Taxable Gallons	Data Enter the number of taxable gallons being reported.
Tax Paid Gallons	Data Enter the number of tax paid gallons being reported.
Interest	Dollar amount Enter the dollar amount of interest.
Surcharge	Dollar amount Enter the surcharge amount, if applicable.
Surcharge Interest	Dollar amount If the surcharge is late, enter the interest accumulated.
Jurs Total	Dollar amount Enter the total amount for each fuel type from this jurisdiction.
